Associate Member Bundle - $3,213.00 (USD) Bundle (up to 15 members) Subscription period: 1 year Automatic renewal (recurring payments) Membership bundle may include up to 14 members + 1 Bundle Coordinator. Members are linked together and managed by the Bundle Coordinator. Bundle members share the same renewal date, status, and membership level. The entire bundle is charged a single membership fee, which is paid by the bundle coordinator. Bundles are used to offer a discounted group membership to companies or teams.

Fellow (CPLCP™) - $400.00 (USD) Subscription period: 1 year Automatic renewal (recurring payments) PM&R Physician must possess a Certified Physician Life Care Planner (CPLCP™) certification from the Certified Physician Life Care Planner (CPLCP™) Certification Board.

Member (CLCP™) - $350.00 (USD) Subscription period: 1 year Automatic renewal (recurring payments) Medical Doctor, Doctor of Osteopathic Medicine, or a non-physician who holds a Certified Life Care Planner (CLCP™) certification, as awarded by the International Commission on Health Care Certification, or a Certified Nurse Life Care Planner, as awarded by the CNLCP® Certification Board.

Associate Member - $300.00 (USD) Subscription period: 1 year Automatic renewal (recurring payments) Other clinical or forensic professionals with an interest in the discipline of life care planning.

Resident - $200.00 (USD) Subscription period: 1 year Automatic renewal (recurring payments) Physician in PM&R residency training program.
